Attention Parents of Incoming Grade 9 Students (YOG 2022)

PHYSICAL EXAM is required for all incoming Grade 9 students.  This Physical Exam must be completed on or anytime after AUGUST 29,2017.  
COPY OF IMMUNIZATIONS is also required for your teen.  Immunizations must be up to date or have a MD appointment note stating when they will receive the needed dose.
MEDICATIONS - New MD order is needed each year for meds to be administered here at school or for student to carry their own Epipen or Inhaler.
DISMISSAL FOR ILLNESS - Student must be seen by school nurse so this absence is excused.  So if they text you, please send them here and remind them to come here first                                                                                                                    
FREE DENTAL CLINIC - This free service is offered in the Fall and Spring.  Please return the permission forms that you will receive this year in the beginning of school packet.
PLEASE KEEP ME APPRAISED of the changing health care needs of your teen.  Any new or changed doses of medications, injuries or new diagnoses.  
Lastly, please keep the main office and I appraised of any change of address, phone numbers or emails.

FREE DENTAL Screening for Students

MidState Dental is again offering free dental exams, cleanings, sealants and temporary fillings.    Permission forms went home at the beginning of the year. Please return the form so your teen may participate!  Please return your permission form by April 15th.  By offering this service, your teen does not miss much of his/her school day and you do not have to take time off from work!  I am attaching the parent permission form in case you need one.  You may have your student bring it to me, email it to me at: bkelly1@sau4.org or fax it to 603-744-2526.  This is a service that is funded by a grant and we hope to continue to offer it but we must show that there is a need for this service.

Physical Exams for students

Attention parents of students in Grade 9 and those that have transferred in from out of state.  NH State Law requires that our school needs to have a copy of a physical exam that was done on or before  8/30/2016.  Please bring in a copy of your teen's physical exam and immunizations before October 13, 2017.  Your MD may fax this information to me at 744-2526.  Clear Choice MD in Belmont, NH offers Sports/School Physical Exams for only $20.  IF this is still a hardship, please contact me.  I need to send a compliance report to the State the middle of October.  Thanks in advance for your co-operation!

END OF YEAR MEDICATION PICK UP

END OF YEAR MEDICATION PICK UP BY PARENT/GUARDIAN - Please remember to come to the Nurse's office to pick up your teen's medications before 11AM on Friday, 6/23/17.  No medications can be stored over the summer.  Medications not picked up with have to be disposed of.  I know how costly this is and I hate to do that but I will not have a choice.  I will have our new district medication forms available to you.  I have also attached them to this email for your convenience.  This should expedite your ability to obtain the MD medication orders for your teen's medications for the 2017/2018 school year.  Have a wonderful, safe, healthy and happy summer.  I look forward to seeing your teen in August!

APRIL DENTAL EXAM/CLEANINGS - Dental Screening and Preventative Services Permission Slips were mailed home with your teen's Quarter 2 Report Cards.  If you would like your teen to receive these services, please return this form to me ASAP.  It is a great opportunity for your child to receive prophylactic dental care while here at school.  This saves you from leaving your job and bringing them to a dentist, as well as, them missing valuable class time.  If you need another form, please notify me or have your child stop by the office.

Spring Dental Screenings

Missing Physicals and Immunizatiions

We have several students not in compliance with the NH State Immunization and Physical Exam laws.  Mr. Hoiriis and I just mailed out letters withing the past 2 days if your child is out of compliance.  I had previously spoken to many parents and left voice mails for those that I could not reach.  Please make every effort to bring your teen into compliance as quickly as possible.  Thanks in advance for your efforts in keeping your teen healthy!  Feel free to call or email me if you have any questions.


From the Nurse's desk 11-10-16

On Tuesday, October 17th, we held our NRHS student's flu clinic here at NRHS.  Sending the paperwork home the first day worked very well.  We had over 68 students sign up for it.  Unfortunately, there were 2 field trips also scheduled for that day.   If your teen was absent or on a field trip that day, they did not receive their flu vaccine here at school.  Please make other arrangements for them.  Your teen should have brought home the  documentation that they received their flu vaccine for your records.  

We had over 60 students sign up for the NRHS Dental Clinic!  Julie Phipps, dental hygienist from Mid-State Dental has done an amazing job!  Dental reports were sent home with each student.  Dental referrals for further care were mailed out within 2 weeks of their visit.  We ended up scheduling these students over a period of 5 days.  I was careful not to schedule your teen during a class that they were struggling with or during their lunch period.  It was quite a challenge!  Friday, November 18th will be the last of the 5 scheduled days for the Fall dental cleanings.  If you did not already sign your teen up for these services, please notify me so you can have  the paperwork filled out to add them to our Spring dental cleanings beginning in April!
