Dental Screening and Preventative Services Permission Slips were mailed home with your teen's Quarter 2 Report Cards. If you would like your teen to receive these services, please return this form to me ASAP. It is a great opportunity for your child to receive prophylactic dental care while here at school. This saves you from leaving your job and bringing them to a dentist, as well as, them missing valuable class time. If you need another form, please notify me or have your child stop by the office.
We have several students not in compliance with the NH State Immunization and Physical Exam laws. Mr. Hoiriis and I just mailed out letters withing the past 2 days if your child is out of compliance. I had previously spoken to many parents and left voice mails for those that I could not reach. Please make every effort to bring your teen into compliance as quickly as possible. On Tuesday, October 17th, we held our NRHS student's flu clinic here at NRHS. Sending the paperwork home the first day worked very well. We had over 68 students sign up for it. Unfortunately, there were 2 field trips also scheduled for that day. If your teen was absent or on a field trip that day, they did not receive their flu vaccine here at school. Please make other arrangements for them. Your teen should have brought home the documentation that they received their flu vaccine for your records.
We had over 60 students sign up for the NRHS Dental Clinic! Julie Phipps, dental hygienist from Mid-State Dental has done an amazing job! Dental reports were sent home with each student. Dental referrals for further care were mailed out within 2 weeks of their visit. We ended up scheduling these students over a period of 5 days. I was careful not to schedule your teen during a class that they were struggling with or during their lunch period. It was quite a challenge! Friday, November 18th will be the last of the 5 scheduled days for the Fall dental cleanings. If you did not already sign your teen up for these services, please notify me so you can have the paperwork filled out to add them to our Spring dental cleanings beginning in April!
FLU CLINIC Permission forms went home on Monday, October 21, 2015. So far only 8 students are taking advantage of this free service. Please check your teen's bookbag for this form. Please have your student return this completed form by Friday, October 23rd, indicating whether or not you want your teen to receive the FLU vaccine. Thank you!
Attention Grade 9 Parents: Please make sure that you have submitted a copy of your child's Immunizations and a recent Physical Exam dated on or after 9/2/14. I need to make a report to the State of NH of students that are out of compliance next month. Students that are not in compliance, need to be excluded from school. Let's work together to prevent this from happening! If you have already scheduled a Physical Exam for your teen, please let me know the date of the exam and that will serve as their exclusion date if the appointment is not kept. Clear Choice MD Urgent Care has locations in Belmont, NH and in Lebanon, NH. It is a walk in clinic and is open 7 days a week from 8AM to 8PM. Cost is only $25. If this is a financial hardship, please notify me so that I can be of help.
FLU CLINIC FOR STUDENTS is scheduled for October 26, 2015. I will be sending forms home soon. Please respond even if you do not want your child to participate. The paperwork will ask for insurance information. Your insurance may be billed, however, there is no cost to you for this service. It is free service for all students.
